Need some measurements for Grover 406C tuners

I'm looking to get some custom made knobs for the Grover 406C tuners. I haven't received the tuners yet and don't expect to get them for at least another month. The luthier needs some measurements before he can start making the buttons. So if somebody has one of these tuners and a vernier caliper handy would you mind taking a look? Here's the message from the luthier:

The measurements I need are shown below. I need:
1) the diameter of the shaft the buttons go over

2) the measurement across the flats on the shaft

3) the length and head type of the screw.
