I would guess on the wiring being wrong and your signal being sent to ground or an air gap in your signal path?
The issue may be with connection to the output jack?
Double check the wiring path and look for frosted solder joints as well.
The traditional way of troubleshooting these issues is to start in the middle and split the problem in half.
Does the the first half work, yes then the problem is in the second half.
Then split it again and repeat.
