    1. Neck pickup
    2. Middle pickup
    3. Bridge pickup
    0. Out to volume (common)
    4. Neck tone
    5. Middle tone
    6. no connection

    (or link 4 & 5 as neck/middle tone and connect to 6 for a bridge tone etc.)

    Ground the switch via tab on the switch body, if not grounded by pickguard shielding.
