I need some help with pickup volume on a PBH-4 Bass

When I just use the neck pick up the volume is great and the same when I just use the bridge pick up, but when I use both together the volume cuts in half. Does anyone have any ideas why this would be happening, the wring diagram I've used is for the PBH-4 I've checked and re- check it, not sure why.

It could be a phase issue so you're getting some cancellation when combining the pickups, or other possibilities. Can you please post some clear photos of the wiring and pot connections etc. as sometimes we can spot something amiss.
